PAYMENT PLAN FOR TUITION for 2010-11 Academic Year

Check one:

o 10 Month (July 1–April 1) $1,099.50/mo.

o 10 Month (July 1–April 1) $1,149.50/mo. (10-11 tuition PLUS 11-12 (next year’s) $500 annual registration fee*)

o 10 Month (July 1–April 1) $_______/mo. based on prior arrangements (i.e., financial grant or home church subsidy). 

If your home church (or other benefactor) is subsidizing your tuition, it is your responsibility to inform the CLS office of the amount and payment schedule of this subsidy and provide written documentation from the church (or other benefactor) regarding this subsidy – including the remittance schedule.  Upon receipt of this documentation, your 10 month payment schedule will be based on the balance due.

o Pay in Full -- $10,665.15 (by June 30 – 3% discount.  Payment must be received in the office of CLS no later than 4:00 p.m. June 30, 2010).

*Annual Registration fee of $500 is payable every February for the upcoming school year unless it is included in your monthly payment (option 2 above)

Please read and initial the following statements:

_____ Monthly Payments: All payments will be automatically withdrawn from a savings or checking account between the 1^st and 10^th of each month.  Please see the tuition link on our website to set up your automatic payment prior to June 15, 2010 OR you may attach a voided check.  Contact our office 7 days prior to a tuition due date to make any change to that month’s automatic payment.  

_____ Prepaid Tuition to Lock in Tuition Rate.   With full tuition payment by July 10^th of the prior academic year, current tuition rate will be locked.  Example:  If you paid two full tuition amounts ($10995 x 2) by July 10^th 2010, there would be a 3% discount for up front payment (hence, total of $21,330.30), and you would be fully paid for the 2011-12 academic year no matter the tuition increase.  Prepayment, with guaranteed tuition fee lock, can occur for up to three future academic years.

_____ Late Payment Charge: Delinquent accounts will be assessed a late charge of $30.00 when full payment of current tuition has not been paid in full by the 10th of each month.  The school may suspend the student(s) if full payment is not received by the last day of the month.  (Please notify the school office of any difficulties.)

_____ Absences: No deduction will be made from the tuition for absences during the school term, regardless of the reason for the absence.

_____ Returned Payments:  A $25.00 charge for all returned payments will be charged.  If this occurs twice, the responsible party will be required to make all future payments in cash, money order, or cashiers check.

_____ Withdraw from school:  If I must withdraw my student, I will give the school two weeks notice.  Should I fail to give such notice, I will be liable for an additional month’s tuition.  With or without notice, should I withdraw my student, I forfeit any tuition, material fees or family assessments that have been paid, unless full payment was made, in that instance tuition will be prorated.

I have read CLSHS’s Belief and Practice Statement and acknowledge that Crean Lutheran High School, in all of its educational and extracurricular settings, operates within this worldview understanding and that I will be supportive and respectful of CLSHS’s commitment to these beliefs, core values, and convictions.

Parent Statement

I give my permission for my child to take part in all school activities, including sports and school-sponsored trips away from the school premises, and absolve the school from liability to me or my child at school or during any school activity.

I understand that the school reserves the right to expel any child who fails to comply with the established regulations and discipline or whose financial obligation remains unpaid after the due date.

I agree to uphold and support the high academic standards of the school by providing a place at home for my child to study and to give my child encouragement in the completion of homework and assignments.

I understand that the standards of CLSHS do not tolerate dishonoring of the Holy Trinity and the Word of God, profanity, obscenity in word or action, disrespect to the personnel of the school or continued disobedience to the established policies of the school.

I agree to authorize the school to employ such discipline as it deems wise, necessary and within reason of the action committed.

Should legal action, for any reason, be taken against Crean Lutheran South High or any employee or agent thereof, on my child’s behalf, CLS will first seek a solution by Christian arbitration.  However, should arbitration fail or should the other party persist in legal action and the school or its agent not be found at fault, I agree to pay any attorney fees, court fees, damages or other costs that CLS or its agent should incur to defend itself against such action.
